Role summary

Job summary
Upper GI/ HPB CNS Working with cancer patients from Diagnosis through to end of treatment and palliative care.
Working independently on own initiative and supporting the wider multidisciplinary team. Conducting Nurse led clinics and supporting Patients, carer throughout the cancer pathway.
Supporting Band 6/7 patient case load , inpatient review and assessment, of patients overseeing effective symptom management and referring to appropriate allied health professionals.

Main duties of the job
Manage and support Patients from diagnostics to cancer treatment, working collaboratively with the Band 6/7, ACP and wider MDT.
Plan, implement and evaluate patient care throughout the pathway.
Clinical admin -Complete referrals to Allied health Professionals. Maintain effective communication within the team locally and cross tertiary with the extended speciality teams , UGI, HPB AND ONCOLOGY
Record keeping, in accordance to Local and NMC Standards. Keep accurate and up to date records.
Patient and peer education, Health promotion
Participate, prep and present patients for Cancer MDT.
Bi weekly inpatient review, - Assessment and care co ordination
