Role summary
The Dementia Link Worker provides targeted care and support for people with dementia and their carers in the London Borough of Hammersmith & Fulham. This role involves linking individuals with appropriate social and health services, conducting ongoing reviews and assessments, and informing patients about dementia. The post holder will liaise with GPs, the West London Health Trust, and other services as needed. Empathy for people with dementia and strong communication skills are essential.
Key responsibilities
- Assessment of individuals with dementia and their families.
- Assessment and management of ongoing risks, in collaboration with service users and carers.
- Work as a member of an MDT.
- Deliver care in partnership with individual service users and their families and carers to ensure the delivery of the care plan.
- Monitor dementia medication, checking concordance and reviewing ongoing efficacy.
- Acting as a link between service users, carers, GP, CIDs and any other relevant services.
Essential criteria
- Educated to GCSE level or equivalent (with a minimum of 5 A-E qualifications, which must include Maths and English)
- NVQ Level 3 in Health and Social Care OR equivalent level of experience/ development
- Significant work experience in health/social care setting
- Experience of working with people with mental health difficulties / knowledge of local resources
- Physical healthcare monitoring and interpretation
- Knowledge of Mental Capacity Act (2005), to include practical knowledge of DOLS and Best Interest decisions
- Knowledge of equality and diversity
- Child and Adult safe guarding procedures.
- Basic computer skills, ability to use 'word, programmes.
- Excellent verbal and written communication skill
- Demonstrate well developed interpersonal skills
- Good at working in a team
- Clear personal and interpersonal boundaries
- Motivation and commitment
- Thoughtful and reflective
- Ability to actively listen and observe
Desirable criteria
- Experience in working with people with dementia on the ward and in the community.
- Lived experience of mental health issues or caring for a person with mental health difficulties
- Knowledge/understanding of strategies relating to Risk management
- Ability to demonstrate emotional resilience and appropriate coping skills

Job summary
The Dementia Link Service provides targeted care and support for people with dementia and their carers, and provides a link between primary and secondary care teams.
The Dementia Link Worker will provide support and information for people with dementia living in the community in the London Borough of Hammersmith & Fulham, and their carers - linking them with appropriate social and health services.
The role involves on-going review and assessment, informing patients about dementia and liaising with GPs, the West London Health Trust and other services as required.
The post holder should have an ability to communicate effectively in writing and verbally. They should have previous experience of working with, and be empathetic to the needs of, people with dementia, older people or other vulnerable people in the community.
