Role summary
This Assistant Practitioner role involves working within a multidisciplinary team in primary care settings, including clients' homes and residential homes. You will provide support for therapeutic and clinical interventions, participate in assessment and planning, and identify appropriate assistive equipment. Key responsibilities
- Assess, plan, and implement therapeutic interventions for clients.
- Provide appropriate, adaptive equipment to meet specific client needs and ensure safety.
- Form professional relationships with clients, family, and carers, communicating condition-related information respectfully.
- Involve clients, family, and carers in care/rehabilitation planning and promote self-management.
- Maintain a caseload of clients, taking responsibility for accepting and discharging them, and undertaking clinical duties autonomously.
- Provide advice, teaching, and instruction to relatives, carers, and other professionals to ensure consistent patient care.
Essential criteria
- GCSE Maths and English
- NVQ 2 or 3 in Health and Social Care and/or significant care experience
- Experience of working in a Health or Social care setting
- Experience of working in community/therapy teams
- Experience of working with adult clients with primarily physical conditions
- An understanding of a wide range of physical conditions or willingness to learn.
- Knowledge and understanding of mental health conditions.
- Be able to undertake frequent standing, kneeling, crouching, hoisting and other moving and handling procedures Regularly move and handle clients that may require moderate physical effort, in confined spaces and often unsuitable environments, using relevant practical experience, equipment and skills that enable optimum safety and best practice.
- A passion to learn new therapy skills to support clients
- Willing to work to meet the needs of the service (to include weekends, bank holidays)
- Holds a full UK Drivers licence and access to a vehicle to use for work purposes
Desirable criteria
- Completed Care Certificate
- Significant experience of client personal, physical and general care tasks
- Ability to complete basic wound care and clinical observations
