Role summary

The Cancer Services department is seeking an enthusiastic and well-organised Patient Pathway Coordinator. This role involves tracking cancer patients through their pathways, monitoring progress against national standards, and collecting/validating clinical data. The successful candidate will work with multi-disciplinary teams to ensure compliance with national Cancer Waiting Times. It requires excellent organisational and communication skills, with a focus on active patient management and data input.

  • Experience of using Windows and Office software including Word, Excel and Outlook

  • Qualifications/training and professional development
  • A good standard of English to GCSE or equivalent

  • Skills, abilities and knowledge
  • Awareness of broader issues within the NHS

Job summary

The Cancer Services department is looking for an enthusiastic and well organised individual to join their team in the role of a Patient Pathway Coordinator.

This is an exciting opportunity to be part of an established service that supports the Trust's Cancer teams. We use the Somerset Cancer Register information system to collect real-time data throughout the patient's referral, diagnostic and treatment pathways. The data aids the clinical management of the patients and informs progress against the national Cancer Waiting Times standards.

The main objectives of the role are:

  • Working with site-specific multi-disciplinary teams to track patients through their cancer pathway.
  • Pro-actively monitor individual cancer patients to ensure that they progress through the pathway within the standard timelines and to prevent breaches of national Cancer Waiting Times standards.
  • Collect and validate clinical data for mandatory cancer datasets.
  • Provide appropriate cover support across the team during periods of annual leave and absence.
