Role summary
This role involves coordinating care for a defined group of service users and their carers, including assessment, planning, and implementation of evidence-based care. The Care Coordinator will manage a caseload, assess new referrals, and develop person-centred care plans. Essential criteria
- Registration Mental Health Nurse RMN Part 3 Mentorship preparation Evidence of continual professional development Meet the requirements for revalidation as set out by the NMC High standard of education Good written and spoken English Registration with Social Work England or HCPC
Desirable criteria
- Evidence of appropriate CPD post-qualification Evidence of further training, e.g. Bsc Community Nursing or AMHP status Clinical skill based qualification e.g. CBT, Psycho social interventions or nurse prescribing.
- Skills and Experience
- Good oral communication skills based on fluency on the English language Literate in IT/Computer Skills Appropriate post registration experience at qualified practitioner level within Mental Health of at least 2 years Experience in the utilization of evidence based assessment tools Excellent communication and listening skills with the ability to tailor the message to a variety of audiences Good Report Writing skills

Job summary
To be responsible as the Care Coordinator for the assessment and coordination of care needs, planning and implementation of evidence based care to a defined group of service users and their carers within a defined catchment area/locality. To provide professional advice and support to other agencies and members of the multidisciplinary team, to act as a role model and resource for junior/less experienced staff.
Main duties of the job
As a Care Coordinator, you will hold a caseload of service users and have a number of responsibilities that include assessment and coordination of care needs. Once needs are identified, you will be expected to plan and implement evidence-based care to the service users and their carers. You will have the support and supervision of your Team Lead, and as a Band 6 clinician, you will be expected to act as a supervisor and resource for junior or less experienced staff.
You will be able to draw on the interventions and expertise provided by your MDT colleagues, such as Support Workers, Social Workers, Occupational Therapists, Psychologists, Employment Specialists, Family Therapists and Medical Colleagues.
