Role summary
This role requires an enhanced clinical practitioner to undertake comprehensive assessments, including history taking and physical examination, for patients with complex, frailty, long-term, palliative, and end-of-life care needs. You will manage chronic diseases, identify deterioration, and implement strategies to prevent acute admissions. The position involves recognizing early symptoms of disease exacerbation and determining when advanced care planning or dying phased care is appropriate. Competence in Level 3 medication reviews and non-medical prescribing is beneficial, or a commitment to undertake the module is required. The role is based in North & East Cornwall and includes travel and night shifts.
Key responsibilities
- Deliver advanced clinical assessment, diagnosis and management for adults with complex, frailty, long-term condition, palliative and end-of-life care needs.
- Develop, implement and evaluate person-centred care plans to support patients to remain safely within their own homes and communities.
- Work collaboratively within a multidisciplinary team and with partner organisations to provide integrated care and prevent avoidable hospital admissions.
- Provide expert clinical decision-making, identifying and responding appropriately to changes in patient condition and escalating concerns when required.
- Undertake Level 3 medication reviews and contribute to medicines optimisation within scope of practice.
- Support advance care planning and the delivery of high-quality palliative and end-of-life care.
Essential criteria
- Substantial experience at band 6 or 7 working with older people, assessing and managing individuals with long term conditions and frailty and people in the last year of life.
- Completion of/or currently undertaking the Degree level long term conditions module, or RCN accredited Nurse Practitioner diploma or the equivalent. Or evidence of undertaking advanced clinical examinations and making independent clinical decisions for complex and unpredictable presentations.
- Current professional registration as an RN or AHP with NMC or HCPC
- Current Non-medical Prescriber or currently /willing to undertake the module

Job summary
The enhanced clinical level of this role will require you to be able to undertake enhanced level comprehensive assessment including history taking, physical examination of the patient and a robust systems review. You should be able to assess and manage frailty and complex chronic disease, identify deterioration and implement immediate strategies to reduce the risk of acute admission. Furthermore you will need to be able to recognise the early symptoms of disease exacerbation, acute illness and injuries, based on an understanding of chronic diseases, the disease process, current evidence and practice standards.
You should be competent to recognise that a person is deteriorating and advanced care planning might be appropriate. You will be expected to recognise that the person is coming into the last days of life and that dying phased care is appropriate.
You should be competent to undertake a level 3 medication review and the non medical prescribing qualification would be beneficial or alternatively a commitment to develop your skills by undertaking the module in due course.
