Role summary
This role is for a Senior Occupational Therapist within the Forensic Community Transitions Pathway, a pilot service supporting individuals transitioning from secure forensic services into community mental health services. The post involves providing specialist assessment, formulation, and treatment to help service users regain skills and confidence for community living. It's a temporary, full-time position until March 2028, offering opportunities to develop leadership and service development skills.
Key responsibilities
- Manage a complex caseload, including specialist assessment, care planning, and care coordination.
- Participate in setting quality standards, auditing, monitoring, and reviewing practice.
- Assess patient's individual care needs, then develop, implement, and evaluate programmes of care.
- Deliver non-pharmacological interventions following specific Occupational Therapy assessments.
- Complete and contribute to robust risk assessments and risk management plans, promoting positive risk-taking.
- Maintain an effective reporting system by observing and reporting on patient conditions.
Essential criteria
- Dip COT or degree in Occupational Therapy plus post qualification knowledge and experience to post master's level as outlined below
- BAOT Registered
- Health And Care Professions Council Registered Occupational Therapist
- Member of Royal College Occupational Therapists
- CPD Folder evidence of ongoing learning/development
- Practice Placement Educator
- Highly developed specialist knowledge across a range of OT interventions and clinical areas
- Evidence of extensive post registration experience with working in forensic settings with adults experiencing mental health and or learning disabilities related difficulties
- Experience of integrating care delivery across multi-disciplinary/multi-agency teams
- Experience of leading/contributing to Multidisciplinary assessment process
- Experience and ability to carry out clinical audit
- Knowledge of other relevant local and national documents/drivers that affect practice
- Knowledge of the wide range of statutory independent and third sector service provision for adults with mental ill health
- Range of specialist assessments and interventions to support occupational performance (e.g. ALLENS, Sensory Assessment/Sensory Integration, VdTMoCA, AMPS, MOHOST)
- Ability to integrate the assessment, care planning and the review process for service users/carers in a sensitive manner
- Range of interpersonal skills, (communication, listening, working collaboratively) maintaining good working relationships with service users/carers, other service providers and agencies

Job summary
The Forensic Community Transitions Pathway is a newly developed pilot service to support people in the Shropshire, Telford and Wrekin area that have left secure forensic services and are transitioning from MPFTs Forensic Intensive Recovery Support Team (FIRST) into a community mental health service. The team will provide specialist assessment, formulation and treatment to support them during this transition to gain/regain their skills and confidence to live successfully in the community.
The service is currently looking for a Senior Occupational Therapist (forensic community mental health specialism) to join the service on a temporary contract until 31/03/2028,full-time post (37.5 hours per week).
This role and service will need to develop strong working relationships with other services both internally and externally and is co-located with other mental health teams as the goals of the service are to support individuals with rehabilitation and re-integration into the community.
This is an excellent opportunity to develop leadership and service development skills and also to understand to wider health and social care system in Shropshire, Telford and Wrekin.
This post is being advertised as a temporary contract until 31/03/2028. However, secondment opportunities would be considered for internal MPFT employees upon prior agreement of your current line manager.
