Role summary

This role is for a registered or soon-to-qualify nurse to join a community nursing team in Peterborough. You will work autonomously within a multi-disciplinary team, providing high-standard holistic nursing care in patients' homes. Key responsibilities

  • Work as an autonomous practitioner within the integrated community nursing team.
  • Be accountable for assessing, interpreting, planning, implementing and evaluating treatment to patients within professional guidelines.
  • Keep the patient at the centre of care, ensuring patient and carer participation in decision making.
  • Provide support and education to peers, new staff, non-registered staff and students.
  • Provide a high standard of nursing intervention within a patient's own home, including lone working.
  • Identify patient needs, agree goals and provide appropriate holistic nursing interventions, referring to other services and ordering equipment.
