Role summary
This role is for a registered or soon-to-qualify nurse to join a community nursing team in Peterborough. You will work autonomously within a multi-disciplinary team, providing high-standard holistic nursing care in patients' homes. The position involves assessing, planning, implementing, and evaluating patient treatment, with opportunities for professional development and supporting peers. A full UK driving licence and access to a vehicle are essential for independent travel.
Key responsibilities
- Work as an autonomous practitioner within the integrated community nursing team.
- Be accountable for assessing, interpreting, planning, implementing and evaluating treatment to patients within professional guidelines.
- Keep the patient at the centre of care, ensuring patient and carer participation in decision making.
- Provide support and education to peers, new staff, non-registered staff and students.
- Provide a high standard of nursing intervention within a patient's own home, including lone working.
- Identify patient needs, agree goals and provide appropriate holistic nursing interventions, referring to other services and ordering equipment.

- Ability to work flexibly as part of a team
- Able to assess plan and implement care and formulate care plans
- Ability to safeguard and promote welfare of children and young people/vulnerable adults

- Has a realistic knowledge of personal strengths and areas for development
- Understands the principle of confidentiality
- Has a range of mechanisms for dealing with stress, can recognise when to use them and does so
- Ability to travel independently around Cambridgeshire and Peterborough area meeting required timescales. A Full driving licence and access to a vehicle is essential.
