Role summary
Join the Urgent Community Response (UCR) team as a Band 6 Practitioner, providing rapid assessments and interventions to keep patients safe at home and prevent hospital admissions. This role involves working with a multidisciplinary team to deliver high-quality, patient-centred care within a 2-hour response time. You will also support the Virtual Wards (Hospital@Home) service, managing acutely ill patients in their homes. A full UK driving license and car use are essential for this fast-paced and rewarding community role.
Essential criteria
- NMC registration
- RN first level nurse (Adults)
- Registered with HCPC/Paramedic 1st level degree
- Teaching and Assessing qualification
- Evidence of continuing professional development to diploma level in a relevant area of specialist practice
Desirable criteria
- Community Nursing Qualification / Nurse Practitioner Qualification
- Trained to interpret microbiology and pathology results
- Case Management
- Experience of working as an autonomous practitioner in either an acute or community environment
- Good understanding of working in partnership with statutory and non statutory agencies to support patient centred care
- Multi-disciplinary and multi agency working
- Proven extensive experience of working with adults/older people
- Experience in a Community setting, or gateway area
- Experience of working between other health and social care organisations /settings
- Ability to interpret blood results

We are seeking a Band 6 Practitioner to provide rapid assessments and interventions, helping to keep people safe and supported in their own homes, avoiding unnecessary hospital admissions.
As part of the Urgent Community Response service, you'll work with a multidisciplinary team to deliver high-quality care to patients in crisis, responding within a 2-hour timeframe. This role is fast-paced, rewarding, and essential to delivering integrated, patient-centred care in the community. Our team also supports clinical delivery of the Virtual Wards (Hospital@Home ) service, providing clinical management to patients with acute illnesses within the place they call home, rather than in a hospital bed.
All applicants must hold a full UK driving license and have use of a car.
Main duties of the job
