Role summary
This role is for a Registered Mental Health Nurse to join a multidisciplinary team within the Community Transformation programme. The practitioner will provide recovery-focused treatment and support to individuals with mental health disorders, from diagnosis to recovery. Key duties include care coordination, risk assessment, and supporting timely hospital discharges. The role requires a positive team player with a special interest in compassionate care.
Key responsibilities
- Work within the MDT and alongside system partners to deliver high quality care and treatment to service users.
- Act as a Care Co-ordinator; formulating care plans, completing/updating risk assessments and collaboratively review care needs.
- Work alongside inpatient services; supporting timely discharges from hospital.
- Fulfil the role of the Duty Worker as part of a service rota.
- Undertake Initial Assessments; delivering comprehensive feedback to the MDT and service users/referrers.
- Provide and oversee the management and care of service users.
Essential criteria
- Evidence of appropriate professional qualification
- Valid UK driving licence
- Minimum 12 months Band 5 experience
Desirable criteria
- Evidence of post qualification CPD
- Own vehicle for work purposes
- Previous work in community mental health setting

As part of our Community Transformation programme, we have an opportunity for a Registered Mental Health Nurse to join our established Multidisciplinary team. The aim of the service is to provide recovery-focused treatment and support via personalised care plans for individuals who are presenting with a range of mental health disorders: supporting them from diagnosis to recovery, to enable them to lead the lives they wish to live. You will work with a Multidisciplinary Team: a spectrum of clinical professionals, which includes Nurses, Psychologists, Support Time Recovery workers, etc: as well as a range of collaborative system partners.
We are looking for a positive team player who has a special interest in working compassionately with individuals and their wider support networks.
