Key responsibilities
- Work with people to rehabilitate in their homes or short-term care facilities.

- Provide education and intervention to allow patients to have tools to self-manage.
- Work in partnership with patients to set goals.
- Hold and manage your own caseload as a specialist clinician.

If you are an experienced Band 5 Occupational Therapist looking to move on from your rotations or an established Band 6 Occupational Therapist looking to work in the community, have you considered joining Ilfracombe Community Rehabilitation Team?
We make a difference to peoples lives and the community by working with people to rehabilitate. This is person centred and takes place in the persons home or in a short-term care facility. The Occupational Therapists and Physiotherapists work together with support team members as one therapy team.
