Role summary
This is a brand-new role within the Neighbourhood Mental Health Team, acting as the first point of contact for patients entering the service. Key responsibilities
- Act as a key link between patients, clinicians and partner agencies, supporting the coordination of new referrals.
- Support clinicians during initial assessments, gather relevant information, coordinate referrals and appointments.
- Provide practical and emotional support to patients and their families throughout the intake process.
- Support clinicians to complete comprehensive new patient assessments through telephone and face-to-face appointments.
- Gather psychosocial, demographic and physical health information to support clinical decision-making.
- Assist in identifying presenting concerns, mental and physical health history, risk factors and immediate support needs.
Essential criteria
- OFQUAL regulated Level 3 or above qualification in a health or social care subject
- Demonstrable experience and knowledge in mental and physical health settings
- A strong understanding of mental health conditions
- Significant experience of working in community support services
- Knowledge of local mental health, learning disability, and safeguarding frameworks
- Experience using electronic health records, systems and digital communication tools
- It is essential to hold a driver's license and access to a vehicle for community-based roles where clients visits are a part of the role.
Desirable criteria
- Professional registration with a relevant body where applicable (e.g., NMC, HCPC)
- Experience working within the NHS or public healthcare setting
- Knowledge of NHS mental health pathways and referral systems
