Role summary
This role is for an enthusiastic and dedicated Heart Failure Clinical Nurse Specialist to join an integrated, multidisciplinary team. The post holder will manage a caseload of acute and chronic heart failure patients, conduct nurse-led clinics, and provide specialist inpatient reviews and education. They will work autonomously across Chelsea & Westminster and West Middlesex University Hospital sites, contributing to service development and quality improvement. Key responsibilities
- Manage a caseload of acute and chronic heart failure patients as an autonomous practitioner.
- Undertake nurse-led heart failure clinics to monitor, review, and optimise patient management.
- Collaborate with the multidisciplinary cardiology team to provide specialist inpatient heart failure reviews and education.
- Maintain the heart failure telephone advice service for patients and healthcare professionals.
- Communicate clearly and effectively with patients, relatives, and healthcare professionals, maintaining high standards of documentation.
- Provide a leadership role and forge links with those involved in the development of cardiology services.
Essential criteria
- RGN Level 1 (A) Evidence of continuing professional development within cardiology. Relevant Degree in nursing or health related field
- Extensive demonstrable experience of working in inpatients and outpatients relevant to cardiology
- Cardiology Experience 2 years
- Relevant clinical experience of assessment and management of Heart Failure
- Experience of managing nurse led clinics
- Experience of nursing involving leadership and teaching
- Ability to use databases and computer literate
Desirable criteria
- Advanced Physical Assessment qualification
- Nurse Prescribing qualification
- Virtual Wards Experience

Job summary
We are looking for an enthusiastic and dedicated nurse to join our integrated, multidisciplinary team to work collaboratively, deliver care meeting best practice in heart failure. It is envisaged this post holder will work with the cross-site team at Chelsea & Westminster site & at West Middlesex University Hospital site.
