Role summary
This part-time Dementia Advisor role supports individuals with a diagnosis of Dementia or Mild Cognitive Impairment across Plymouth. Based at Mount Gould Hospital, the advisor will provide group or individual support to help people and their families develop support plans. The aim is to maintain independence, improve well-being, and enable them to live well with their diagnosis through educational programmes, guidance, and collaboration with other agencies.
Key responsibilities
- Manage referrals coming into the service in liaison with service administration staff.
- Support planning of and participate in post diagnostic group programmes and stand-alone education sessions.
- Work with individuals with a diagnosis of a Dementia or Mild Cognitive Impairment and their families and/or carers, in the development of a support plan.
- Provide information regarding services and available support and where needed to help with access to these.
- Maintain accurate and timely electronic and written records in relation to contact with users of the service.
- Participate in service development and planning.
Essential criteria
- Good organisational and time management skills.
- Good written and verbal communication skills.
- Basic IT skills inc. Microsoft.
- Experience working with people with Dementia and their carers.
- Experience of working with statutory and voluntary agencies.
- Experience of record keeping and report writing.
- A good understanding of dementia.
- An understanding of the needs of people with dementia and their family and/or carers.
- An understanding of confidentiality.
- Knowledge of relevant legislation including the Health & Safety, the Mental Capacity Act and the Care Act.
- Ability to work effectively as part of a team and on own initiative.
- Ability to travel around city of Plymouth to a range of locations with appropriate equipment such as teaching equipment/resources and laptop computer.
- Vocational Qualification Level 3 or evidence of equivalent experience.
- Maths and English GCSE Grades 4 and above or equivalent.
Desirable criteria
- Previous use of clinical systems e.g. Systmone
- Experience of presentation within a group setting
- Has attended training in relation to Dementia.

The Dementia Advisor Service are looking for an enthusiastic part-time Dementia Advisor to support people across the Plymouth area who have a diagnosis of Dementia or Mild Cognitive Impairment.
The role will be
based at Mount Gould Hospital and will provide
group or individual support across the Plymouth area to help people with a Dementia or Mild Cognitive Impairment diagnosis, to develop a plan to support them
and their families and/or carers in meeting their needs, enabling people to
maintain as much independence as possible, in a place of their choosing, in a
