Role summary
This role supports the Patient Safety Specialists Nursing Officer in implementing and coordinating clinical risk management activities within the Trust. The Patient Safety Investigator will work with Clinical Divisions to undertake patient safety incident investigations and reviews. This involves reporting, investigation, recommendations, and ensuring practice changes are implemented. The post holder will also process routine enquiries, maintain incident documentation, and ensure transparency and the Duty of Candour are upheld.
Key responsibilities
- Organise own workload and maintain clinical knowledge for Patient Safety Investigations.
- Collate statements, evidence, and information to write robust investigation reports.
- Maintain up-to-date electronic records of investigations and organise regular team meetings.
- Ensure open, honest, and transparent communication with patients, relatives, carers, and staff during investigations.
- Support outcomes and learning responses following patient safety incidents, including monitoring safety improvement plans.
- Roll out training programmes on patient safety incident investigations and Duty of Candour to Trust staff.
Essential criteria
- Educated to degree level or with relevant equivalent experience
- Demonstration of ongoing professional development
- A firm understanding of patient confidentiality, how to maintain it and laws related to it
- Experience of coordinating investigations (i.e. HR, complaint, internal review of service investigations).
- Experience of working within the acute healthcare setting
- Understanding of the principles of the Patient Safety Incident Response Framework (PSIRF)
- Ability to communicate with colleagues at all levels in the organisation including Trust Directors and Divisional Leadership Teams
- Able to present and receive factual information within a multi-professional team
- Team player who can initiate and complete projects and support with improvement plans
- Ability to generate, report and present data to all levels of Trust personnel.
- Demonstrate self-awareness and understanding of personal accountability
- Demonstrate insight in the use of data to influence patient care and services
- Able to work autonomously
Desirable criteria
- Qualification in patient safety
- Experience of managing patient safety incidents, including Never Events
- Experience of difficult and/or emotive discussions with patients, families, staff or other individuals
- Experience of providing formal teaching/training sessions
- Understanding of HSSIB

Job summary
To support and deputise (on a rotation basis with other Band 7's) for the Patient safety Specialists Nursing Officer in implementing, monitoring and co-ordinating all aspects of risk management activity, specifically clinical risk management within the Trust ensuring that systems and processes are in place for continuous improvement in quality and safety.
The Patient Safety Investigator is a member of the Care Quality Patient Safety Investigation Team, however this role will be working with the Clinical Divisions to support the** Patient Safety Divisional Lead** Investigator in undertaking Patient Safety Incident Investigations and Patient Safety reviews.
Main duties of the job
This includes reporting, investigation/review, recommendations, learning responses and ensuring that any practice changes that are required are carried out according to the recommendation contained with the review reports.
