Role summary
This role involves working as part of a community nursing service across the Fylde coast, overseeing and supervising nursing interventions in patients' homes or clinic settings. The postholder will manage a caseload of patients with complex needs, providing safe, effective, and individualised care. Key responsibilities include assessing, planning, and evaluating patient requirements, implementing treatment regimes, and providing leadership and operational management to a community nursing team. Essential criteria
- Clinical qualification to degree level or equivalent/Registered nurse
- Training and experience in clinical management
- Evidence of ongoing professional development
- Current NMC registration
- V300 Non-medical prescriber or willingness to complete
- Community Specialist Practitioner qualification
- Training & experience in clinical management
- 2 years experience at band 6 in the community
- Experience in leading at senior level
- Experience in change management & implementing change
- Able to deal with difficult situations
- Involvement in complaint & investigation management
- Ability to Identify and assess patients' complex needs.
- Demonstrate an understanding of clinical governance & safety/quality initiatives pertaining to the role
- Excellent communication skills
