An exciting opportunity has arisen for an experienced RSW to join our Community Rehabilitation Team (CRT) in the Kirkcaldy Locality.

The CRT provide rehabilitation to the adult population within the locality. This will be delivered to service users as in-patients, out-patients or within their own home.

The primary function of this role will be to work with the Occupational Therapy, Physiotherapy & District Nursing Services within Intermediate Care to deliver therapeutic treatment programmes to service users in the ward setting or their own homes, preventing hospital admission and supporting early discharge from hospital. Occupational Therapists, Physiotherapists & District Nursing will provide specialist assessments to this patient group and allocate work to this post holder. This post will primarily deal with the function of daily care in the community, a service provided 365 days of the year from 0745-2200.
