Role summary

Key responsibilities

  • Optimise patient medication to identify and resolve medication-related issues.
  • Reduce risks from prescribing errors and continuously review prescribing for harm-free patient care.
  • Work closely with healthcare professionals, including specialist nurses and consultants, within multidisciplinary teams.
  • Provide expert advice on medication management and support medical staff in safe, cost-effective prescribing.
  • Ensure accurate medication information is provided to primary and community care providers upon patient discharge.
  • Be willing to work flexible hours over a seven-day period within a structured rota.

Essential criteria

  • Vocational Masters degree in Pharmacy or equivalent

This position is ideal for pharmacists looking to develop clinical skills in a dynamic working environment, supported by our senior pharmacists.
