Role summary

This is an exciting opportunity for a Band 5 Community Staff Nurse to join the dedicated Ross-On-Wye District Nursing Team. The role involves providing holistic nursing care and support to patients in their own homes, aiming to encourage independence and prevent hospital admissions. Key duties include palliative care, wound care, managing long-term conditions, and performing holistic assessments. The team works closely with multi-disciplinary teams and provides a wound clinic service.

Job summary

We have an exciting opportunity for the right candidates for a Band 5 Community Staff Nurse post.

The key principles of Wye Valley Trust are to improve the health and well-being of the people we serve in Herefordshire and surrounding areas. We have an exciting opportunity for the right candidate to join our wonderful District Nursing Team.

The Ross-On-wye District Nursing Team are a very supportive and dedicated team, we are an incredibly engaging, positive and hard-working team, providing care and support to patients in their own homes.

We work closely with Golden Valley DN Team and often work as a locality where needed.

We work closely with a wide range of multi-disciplinary teams, including GP's, Palliative Care Nurses, Therapy Teams, and Advanced Clinical Practitioners. Ross DN Team also provide a wound clinic service based at the Community Hospital.

Main duties of the job

Our aim is to provide treatment and support to patients in their own homes. Whilst undertaking a holistic nursing assessment, we aim to encourage patients to remain as independent as possible, in order to prevent hospital admissions.

Due to the nature of this role, lone working and the ability to meet the travel requirements of the post are essential.
