Role summary
This is an exciting opportunity for a Band 5 Neonatal Homecare Nurse to join the Neonatal Homecare Service, providing specialist on-going care for neonates and support to families in their home environment across Leicester and Northamptonshire. The role involves ensuring high standards of clinical care, evidence-based practice, and seamless discharge from hospital. The service operates 7 days a week, from 08:00-17:00, and includes home phototherapy, oxygen, and nasogastric tube feeding. The successful candidate will be a highly motivated Neonatal Nurse, ideally with post-specialist neonatal course experience or community care background.
Key responsibilities
- Assist with early and appropriate discharge planning in accordance with outreach care criteria.
- Provide seamless and timely discharge from hospital to the home environment, including parent education programmes like Basic Life Support.
- Work without direct supervision to assess care needs, plan, organise, and implement home care, ensuring continuous integrated care.
- Provide on-going holistic care for vulnerable neonates and their families in the community setting.
- Monitor nutrition, feeding skills, and growth, making onward referrals and identifying interventions when required.
- Develop and facilitate an evidence-based learning culture, supporting new starters and students.
- Recognise, prioritise, and respond appropriately to urgent and emergency situations.
- Take responsibility for the care needs of individual allocated patients, referring to Band 6/7 for advice when necessary.
- Provide evidence of work diary, identifying planned visits in accordance with lone worker policy.
- Monitor and inform of relevant changes to care plans required to meet health needs, escalating changes to condition appropriately.
- Work as part of a team, providing cross-site cover as required.
- Take a proactive role in the education and training of multidisciplinary teams, providing supervision and support for neonates requiring on-going outreach care.
- Work effectively as an Outreach team member and independently in planning, assessing, implementing, and evaluating neonatal community care.
- Report safeguarding concerns, follow safeguarding advice, and initiate action/referrals where appropriate.
- Be confident and competent as a lone worker within the Outreach setting in accordance with trust policy.
- Ensure fully accurate, complete, and contemporaneous nursing records are written and maintained for each patient.

- Registered Nurse Child , Adult branch/ Midwife

- QIS in Neonatal Care
- Post registration training in community care of the neonate
- Neonatal Foundations course
- Teaching and assessing qualification
- Minimum of 2 years Neonatal (or relevant post registration
- Experience eg Health Visiting or Community Paediatrics
- Community experience
- Communication and relationship skills
- Good standard of spoken and written English
- Good listening and communication skills
- Diplomacy
- Analytical and Judgement skills
- Effective decision making and problem solving
