Role summary
This role involves working as a Nurse Practitioner within the Ageing Well and Frailty Service, supporting people with moderate and severe frailty. You will collaborate with community care staff and Primary Care Network colleagues to proactively manage patient care. The position requires developing and implementing patient-specific care plans, ensuring clinical standards are maintained, and working both autonomously and as part of a team. The aim is to improve the quality of life for patients with complex needs and reduce pressure on unplanned care.
Essential criteria
- Registered General Nurse educated to degree level (or equivalent)
- Nurse Prescriber or willing to undertake
- Specialist Practitioner Qualification - District Nursing (or equivalent postgraduate modules, nursing qualification/ experience
- Care of the Older Person or similar course (or willing to undertake)
- Evidence of further recent study/ professional development
- Leadership qualification - ILM level 6 or equivalent (or willing to work towards) or relevant equivalent experience; mentorship
Desirable criteria
- Minimum 3 years post registration experience
- Experience and Knowledge
- Knowledge/experience of working with older people
- Knowledge of vulnerable adult's procedures and safeguarding
- Community Nursing experience
- Experience of clinical assessment, analysis, planning, delivery and evaluation skills
- Experience of managing complex and challenging cases
- An excellent understanding of Frailty and applying this to practice
- Specialist knowledge of the effects of the ageing process
- Professional awareness of development and innovations in Community Nursing
- Knowledge of national agendas and the effect on practice eg NHS Long Term Plan and National Service Framework
- Knowledge and understanding of Comprehensive Geriatric Assessment
- Knowledge of local health needs strategies
- Experience of managing a caseload
- Knowledge of services available for older people in the community and the referral process
- Knowledge and experience of Emergency Health Care Planning
- Excellent knowledge of End of Life care including GSF Prognostic Indicators and knowledge of Advanced Care Planning
- Knowledge and experience of managing people living with Dementia and their carers
- Knowledge and understanding of the Frailty register

Job summary
We are recruiting to a team of dedicated community nursing staff to work with the Ageing Well and Frailty Service and our Primary Care Networks. This exciting opportunity means that you'll be working as part of a combined team of community care staff and PCN colleagues to proactively support people with moderate and severe frailty as part of WCHC's Population Health Management and NHS Proactive Care model.
You will be based within the Ageing Well and Frailty Service service working 22.5 hours per week over 3 days.

Main duties of the job
This post requires good communication and interpersonal skills and the ability to be adaptable, reliable and approachable.
You will be working with the patients, their family/carers and the wider team (including PCN colleagues) to create outcome goals for the patient to achieve during their time actively within the service, and lead on the development, implementation and monitoring of the patient specific care plan to ensure thesegoals are achieved.You will be working in both lone capacity and in conjunction with other team members, ensuring clinical standards are maintained using evidence based practice.
