Role summary
This is a community-based role for a caring, motivated, and dynamic nurse to join the Bodmin & Three Harbours Community Nursing Team. You will be responsible for assessing, planning, implementing, and evaluating care for patients, carers, and relatives in their homes, working autonomously to promote independence and quality of life. The role requires close collaboration with GPs, health and social care colleagues, and frequent travel within the area. - Registered Nurse (Adult)
- Evidence of recent relevant professional development
- To act as a mentor for student university placements. Having the mentoring qualification or being willing to work towards this qualification and attending mentorship updates as required.
- Current Professional Registration - NMC

- Hold a post-registration diploma or be working towards a degree
- Skills and Aptitude
- IT skills to include use of Word, Excel, email, internet, recording systems
- The ability to provide skilled, evidence based nursing care to a wide range of patients
- Ability to deal with patients and carers who are under stress.
- Awareness of and adherence to policies including lone worker and manual handling policies
- Current knowledge of developments in nursing and healthcare in general.

You will be part of the Bodmin & Three Harbours Community Nursing Team and you will be based at Bodmin Health Office, covering Bodmin, Par, Lostwithiel and the surrounding areas.We cover Bosvena and Three Harbours practices within the local PCN. Your role will require close collaboration with GP's, health and social care colleagues including therapists and voluntary organisations.
