Role summary
This role is for a Registered Nursing Associate to join the Primary Care Home (PCH) Team in the Waveney area, providing person-centered care to adults in their homes, residential care, or clinic settings. The Nursing Associate will work independently under the leadership of Registered Nurses, managing a delegated caseload and performing various nursing and reablement interventions. The team operates 7 days a week, with rotational shifts, and offers preceptorship for newly qualified individuals. The aim is to support patients to maintain health or recover from illness at home.
Essential criteria
- *Registered Nursing Associate on the NMC register
- *Foundation Degree
- *GCSE Maths and English grade 4-9 or functional skills level 2
- *Health coaching or willingness to undertake
- *Previous experience of working in health care

- *Ability to work on own initiative
- *Ability to organise and prioritise own delegated workload
- *Able to recognise own limitations in the context of the Nursing Associate role

Job summary
We are ready to welcome another Registered Nursing Associate to the Primary Care Home (PCH) Team in the Waveney area which includes Beccles, Bungay, Halesworth , Southwold and Kessingland.
Do you want to develop and grow your clinical practice in an encouraging and supportive team? Do you like variety, want to work where no two days are the same? If yes, this role could be for you. The Nursing Associate will work independently under the leadership of Registered Nurses, working within the sphere of nursing and care and within all aspects of the nursing process. The PCH is a multidisciplinary team delivering health care to people in Waveney. The combined skills of the team mean they are able to respond to the changing needs of patients and support those who may previously been admitted to hospital to remain at home. You will become an active member within this team of clinicians. We would be very happy to provide preceptorship to a newly qualified Nursing Associate
The PCH team provides community services 7 days per week 07:00- 20:30 hours
