Role summary
This role is for a compassionate and motivated Registered Nurse to join the Out of Hours Community Nursing Service in East Cheshire. You will provide high-quality, patient-centred care in patients' homes and residential settings, delivering a wide range of nursing interventions to help patients remain safely at home. The position offers flexible working patterns, including evening shifts, and focuses on developing clinical skills and autonomy within a supportive multidisciplinary team.
Essential criteria
- Registered Nurse to degree/diploma level
- Current registration with NMC
- Evidence of recent professional development in an up-to-date portfolio
Desirable criteria
- Non- Medical Prescriber Teaching/Assessing qualification
- Recent previous experience within a comparable role/transferable skills
- Demonstrable knowledge of assessment and therapeutic interventions in area of practice
- Experience of developing programmes of care for an individual or groups of patients/clients and of providing advice
- Ability to prioritise and organise workload effectively
- Experience of using electronic patient/service user record systems
- Ability to communicate in a professional way with people at all levels of understanding either face to face, on the telephone, e-mail or written communication
- Compassionate - with exceptional interpersonal skills with the ability to communicate effectively with staff, patients, carers and relatives (as appropriate) remaining sensitive and empathetic
- Experience of mentoring students / supervise other junior staff
- Experience of working within a multi-disciplinary team.
- Previous experience delivering care in a community setting.
- Experience of working in a rehabilitation environment.
- Experience of working in multi-agency services. Experience of mentoring Knowledge and experience relevant to area of practice
- Understanding and / or experience of interagency working
- Knowledge and skills
- Ability to assess clinical situations and act appropriately
- Able to assess, plan and evaluate patient care, considering the needs and desires of the patients
- Awareness of effects of ill health and the ageing process
- Awareness of the principles and practice of SSKIN.
- Computer literacy including Knowledge of Microsoft office systems and use of email
- Leg ulcer assessment and management, Experience in palliative care, end of life care.
- Understanding of Gold Standard Framework
- Diabetes care/management.
- Understanding and/or experience of managing patients with LTC needs
- Specific Job Requirement
- Must be able to undertake all aspects of the role whilst working as a lone worker
- Able to organise own workload and deal with unpredictable nature of the role
- Ability to travel across the county with heavy / bulky equipment and the ability to travel from patient to patient, whilst maintaining a consistent caseload

We are seeking enthusiastic Band 5 Community Staff Nurses to join our well-established Out of Hours Community Nursing Service, providing high-quality care across East Cheshire.
As part of our supportive and experienced team, you will deliver a wide range of nursing interventions, helping patients remain safely at home and avoiding unnecessary hospital admissions. No two shifts are the same, offering opportunities to develop your clinical skills, autonomy, and decision-making in a diverse community setting.
We understand the importance of work-life balance and can offer a flexible approach to working patterns. Whether you are looking for just one shift per week or several shifts, we would be delighted to hear from you. We currently have a range of evening shifts available throughout the week, including 18:00 to 22:00 and 17:00 to 22:00 shifts, enabling you to fit community nursing around your existing commitments.
