Role summary
This role is for a Band 6 Mental Health Practitioner within the Crisis Resolution & Home Treatment team (CRHT). You will be responsible for assessing care needs, developing and evaluating care programmes, and providing intensive home support as an alternative to hospital admission. The position involves working on a rotational shift basis, including weekends and night shifts, across Halton, Warrington, St Helens & Knowsley. You will manage a caseload, conduct specialist assessments, and collaborate with multidisciplinary teams to support service users with complex needs.
Key responsibilities
- Formulate, contribute to, and participate in, the development and implementation of assessment and care planning systems.
- Manage a case load within the Crisis Resolution Home Treatment Team.
- Carry out generic and specialist assessments, including risk, and provide therapeutic interventions/home treatments.
- Complete Gatekeeping for all Inpatient admission requests across the Merseyside footprint.
- Act as the Duty Practitioner /2nd Duty, as part of a rotational roster.
- Ensure record keeping is in accordance with Professional and Trust standards.
Essential criteria
- Appropriate professional qualification (e.g. RMN, AHP, Social Worker); Evidence of post-registration CPD
- Knowledge of mental health legislation, CPA, safeguarding, equality and human rights, clinical governance and professional codes of conduct.
- Working within multidisciplinary teams.
- Knowledge of safeguarding procedures and crisis intervention.
