Role summary
This Specialist Nurse role is for Transcend, a new service supporting Gender Incongruence within Primary Care. The postholder will provide direct clinical nursing care, manage a patient caseload, and work closely with an MDT, GPs, and partner agencies. It's an opportunity to contribute to a new model of care for an underserved group, requiring autonomous work and travel.
Essential criteria
- First level Registered Nurse/Midwife
- Ability to be a clinical mentor (ENB 998/997) or equivalent post registration degree level module or willingness to undertake
Desirable criteria
- Working towards Gender Identity Healthcare qualifications via RCP
- Completion of Management/leadership course
- Knowledge/Experience
- Previous experience of working with Gender Incongruent service users
- Aware of protocols for administering of GRNH analogues
- Use of EMIS as a reporting tool
- Organised
- Ability to travel
- Excellent communication with colleagues and service users
- Administering IM injections
- Interpreting blood results
- Excellent documentation skills
- Ability to assess and gain information in short timescales
- Ability to build relationships quickly

Job summary
Transcend is a newly commissioned service established to explore how Gender Incongruence can be best supported and treated within a Primary Care setting. We are seeking a Specialist Nurse to help deliver this care. The postholder will work closely with our MDT, local GPs, relevant partner agencies, and of course directly with service users.This is an exciting opportunity to contribute to the development of not only a new service, but a new model of care for a disadvantaged and underserved service user group. The role will involve travel across the locality, therefore being a car user is essential.
Main duties of the job
To provide direct clinical nursing care to the service users of Transcend. This will include both direct and indirect interventions to support individuals accessing the service. The ability to work autonomously, as well as part of a wider multidisciplinary team, is essential. The postholder will be responsible for managing a caseload of patients and supporting them throughout their journey with the service.
