Role summary

Key responsibilities
- Manage daily operations of caseload and triage referrals for admission.
- Support staff with patient escalation, troubleshoot, safety net, and make clinical decisions to keep patients safe at home.
- Provide high quality nursing care and participate in the assessment, planning, implementation and evaluation of care needs.
- Deliver compassionate person-centred care, treating patients, service users, their families and carers with dignity and respect.
- Provide relevant forms of care with minimal supervision and participate in the supervision, teaching, and assessment of unregistered or junior staff and students.
- Administer IV medications to patients in a home environment and clinic/outpatient setting to avoid hospital admission.
Essential criteria
- Acute NHS hospital experience as a registered nurse
- Community experience
- NMC Qualification
- FULL UK Driving license and access to own vehicle with business insurance (cover for driving for work)
- Registered Nursing experience of at least 6-12 months
- Competent in IV administration
- Experience in long term conditions
- Strong IT skills
Desirable criteria
- Competent in use of longlines (PICC, Hickman, smart-mid)
- Cannulation and venepuncture
- Degree module for long term conditions ie respiratory, asthma, diabetes
- Health assessment module
