Role summary
This fixed-term role involves providing person-centred therapeutic support to older people with mental health difficulties across community settings, care homes, and memory assessment services. You will conduct specialist assessments, reviews, and interventions for complex needs, including dementia and cognitive impairment. The role requires managing a varied caseload, supporting timely diagnosis, and responding to crises. You will collaborate with a multidisciplinary team and contribute to service development and quality improvement.
Key responsibilities
- Provide person-centred therapeutic support to older people experiencing mental health difficulties.
- Carry out specialist assessments, reviews, and interventions for people with complex needs.
- Manage a varied caseload across community and care home settings.
- Deliver practical, evidence-based care for people with dementia and other mental health conditions.
- Support timely diagnosis, manage risk, and respond appropriately to crises.
- Collaborate with GPs, OTs, psychologists, and social care colleagues to ensure coordinated care.
Essential criteria
- Knowledge of Functional Mental Health Concerns
- Knowledge of Dementia Types
- Knowledge of Mental Health Act and Mental Health Measures
- Knowledge of Medications used in our practice
- NMC Registration
Desirable criteria
- Qualified for 1 year at least having completed preceptorship program
- Experience within Older Adult Mental Health
- Ability to be able to travel to different locations in the borough

Job summary
This post is a fixed term/secondment for 6 months from start date to meet the needs of the service. If you are interested in applying for the secondment position, you must obtain permission from your current line manager prior to applying for this post.
In this role, you'll provide person-centred therapeutic support to older people experiencing mental health difficulties across community settings, care homes, and memory assessment services.
You'll carry out specialist assessments, reviews, and interventions for people with complex needs, including dementia, cognitive impairment, and other mental health conditions, making sure care is effective, evidence-based, and as least restrictive as possible.
You'll work flexibly across services--supporting patients, carers, and staff while also getting involved in service development and quality improvement.
You'll also help plan and oversee care for patients in both primary and secondary care, including those living in the community and in care homes.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
