Role summary
This role involves providing mental health assessment, treatment, and support to people in Telford & Wrekin as part of the Urgent Care Pathway. Working within a multi-disciplinary team, you will conduct specialist assessments, develop care plans with service users and carers, and evaluate treatment outcomes. The team operates 24/7, supporting individuals experiencing mental health crises or requiring home-based treatment.
Essential criteria
- A recorded/registered qualification in one of the following in CMHN, Social Work, Occupational Therapy, Psychological Therapy
- Post qualification experience, such as evidence of formal courses and training relevant to people with complex mental health needs related to emotional instability
Desirable criteria
- Qualification in psychological interventions such as DBT, SCM, CBT, EMDR, MBT, relevant to a client group with complex mental health needs
- Experience of working with a client group who present with difficulties relating to emotional dysregulation, high risk challenging and chaotic behaviour, highly complex needs relating to trauma
- Experience of working with people who have difficulty in engaging with services
- Experience of working with people engaging in high risk behaviours and self harm
- Experience of working within a structured therapeutic model individually and in group settings
- Experience of facilitating psycho-educational and/or skills based groups

The team is based at Hall Court in Telford and provides mental health assessment, treatment and support to people in Telford & Wrekin. As an integral team in the Urgent Care Pathway we work 24 hours a day, 7 days a week. We work with people who are experiencing a mental health crisis, require extra support alongside their regular support network or who can have the same treatment provided at home that they would receive in a Mental Health Hospital.
We are a team that consists of Band 5 and 6 Nurses, Band 3 Support Workers, Non-Medical Prescribers, Medical Staff, Psychologists, Occupational Therapists, Social Workers, Administrators and Business Support Staff.
As we work with people with all types of mental health presentations, we need to have a range of skills and interests that create a strong and responsive team.
Main duties of the job
Mental Health Practitioner - Nurse, OT, Social Workers
Working in a multi-disciplinary team and in partnership with internal and external agencies, you will;
Provide specialist assessment and care planning in conjunction with service users and carers
Evaluate treatment plans and undertake home treatment
Triage and assess referrals received by the service in liais
