Role summary
This role is for an experienced Senior Community Physiotherapist to join a supportive team providing domiciliary rehabilitation services. You will treat a diverse range of conditions including elderly care, falls, orthopaedic, neurological, and respiratory issues. The position involves managing your own caseload, working autonomously within a multidisciplinary team, and developing leadership skills. The aim is to prevent hospital admissions, promote independence, and facilitate timely discharges.
Essential criteria
- \*Appropriate professional qualification (Occupational Therapist or Physiotherapist) with full HCPC registration
- \*Degree level award (level 6) plus post graduate specialist training, experience, short courses to masters equivalent level.
- \*Able to demonstrate a good command of the English Language both written and verbal
- \*Computer Literate in both data entry and communicating via IT - emails and report writing
Desirable criteria
- \*Further education training eg: external courses/masters degree or equivalent experience gained by undertaking on-going personal development and training.
- \*Management/Leadership Qualification/development programme
- \*Experience, underpinned by knowledge of working with and understanding the complex needs of patients in a primary care/community setting.
- \*Have a broad range of clinical skills, including assessment and treatments of a variety of conditions including neuro, orthopaedic, MSK and elderly.
- \*Involved in the implementation and management of change.
- \*Experience of working with long-term conditions and frailty.
- \*Demonstrates organisational skills: including the ability to make decisions and to prioritise.
- \*Ability to understand and interpret research findings/evidence-based care and apply to practice.
- \*Participation and comprehension of clinical and professional supervision.
- \*Experience of initiating or participating in quality improvement project relating to clinical practice

Job summary
Senior Community Physiotherapist - 22.5 hours per week
We are seeking an experienced Physiotherapist to join us. The North Hampshire Community Therapy Team is a friendly and supportive team of Community Therapists and Therapy assistants providing a domiciliary rehabilitation service to patients. You will develop your expertise treating a range of problems related to elderly care, falls, palliative care, orthopaedic, neurological and respiratory conditions. You will also have opportunity to develop your leadership and management skills within the team.
This post would suit an established B6 therapist or an experienced B5 with an interest in community working. Successful applicants will need to be an efficient worker, a good communicator and be confident working as an autonomous practitioner. Community experience is desirable. You will be responsible for managing your own caseload whilst working effectively within the multidisciplinary team to ensure an integrated approach to patient care - preventing unnecessary admission to hospital or long-term care, promoting functional independence and facilitating timely discharge from hospital.
