Role summary
This is an exciting opportunity for a highly motivated and creative Highly Specialist Physiotherapist to join the Learning Disabilities Service at Mersey Care NHS Foundation Trust. Based with the Halton Community Learning Disability Team, you will provide specialist physiotherapy assessment and intervention for service users with learning disabilities and complex needs. The role involves supporting and contributing to physiotherapy and learning disabilities strategic and team development, working within an established multidisciplinary team. You will manage a complex caseload autonomously, using evidence-based principles to assess, plan, implement, and evaluate interventions.
Key responsibilities
- Provide expert clinical leadership and implement best practice interventions within your specialism.
- Undertake timely, holistic assessments of patients, including functional and environmental factors.
- Effectively manage a specialist and complex caseload as an autonomous practitioner, using evidence-based principles.
- Plan, develop, and evaluate Physiotherapy Services within a designated area/team.
- Lead and motivate a team, providing clinical supervision and contributing to staff development.
- Participate in clinical governance, audit, and service development projects.
Essential criteria
- Diploma/Degree in Physiotherapy
- Current registration with HCPC as a Physiotherapist
- Experience and/or additional education in relevant field to post graduate diploma or equivalent level, combination of education, further training and experience
- Evidence of CPD relevant to treatment of older adults, physical disability and rehabilitation
- Experience and /or additional education within your specialism Evidence of an up-to date CPD portfolio
- Evidence of recent post graduate experience in your specialism.
- Experience of working in a community or primary care setting.
- Experience of negotiating and liaising within a multidisciplinary/ interdisciplinary environment.
- Experience of working and assessing people with complex needs and delivering complex care plans.
- Experience of coordinating and managing a team.
- Experience of supervision and teaching others.
- Active participation in service training.
- Experience in service development.
- Knowledge of principles of care for long term conditions, relevant National Service Frameworks, NICE and other national standards including the out of hospital strategies
- Knowledge and understanding of current health care policy and relevance to the delivery of health care services.
- Knowledge of Primary Health Care and collaborative working, e.g., social care, acute hospitals and voluntary services.
- Knowledge of the audit cycle.
- Knowledge of evidence based practice.
- Knowledge and understanding of clinical governance.
- Knowledge of how equal opportunities can be implemented in practice.
- An Understanding of the importance of involving service users in their treatment and development of the service.
- Advanced skills in the assessment and treatment of adults with complex issues in relation to specialism and education

Desirable criteria
- Membership of the Chartered Society of Physiotherapy (CSP).

- Experience of working with clients requiring intermediate care if relevant.
- Experience of working with in a virtual ward model in a community setting if relevant.

Being part of Mersey Care NHS Foundation Trust, you will be given opportunity to network across different learning disabilities services and liaise with other PT's and AHP's within the wider Trust linking into the AHP workforce.
