Role summary

This Assistant Practitioner role is within the Wyre Forest Reablement Team, focusing on rehabilitation for patients discharged from hospital or needing increased care in the community. You will work under the guidance of qualified therapists, managing a caseload of patients and delivering person-centred interventions to promote independence. The role involves working closely with a multidisciplinary team including physiotherapists, occupational therapists, and social care colleagues. This is a part-time post of 30 hours per week.

Essential criteria

  • Significant experience working within a relevant clinical service as a senior HCA / HCSW.

Desirable criteria

  • Experience of working at as an Assistant Practitioner or therapy assistant within a relevant clinical service.
  • Additional Criteria
  • Ability to travel independently across the county
  • I understand that the Trust will not routinely provide sponsorship to work for this role, and employment on a time limited visa doesn't guarantee sponsorship at point of expiry.
  • Working knowledge of Assistant Practitioner role within a rehabilitation setting.
  • Understanding of the competency boundaries of an Assistant Practitioner role.
  • Has completed Foundation Degree modules within a directly relevant clinical field.
  • Skills and Abilities
  • Role modelling good behaviour.
  • Ability to challenge poor behaviour.
  • A positive attitude and able to act on feedback.
  • Ability to use IT confidently and competently in daily work.
  • Foundation Degree in Health and Social Care (or alternative equivalent award e.g. Higher National Diploma (HND) or BTEC (level 4).
  • GCSE Maths and English (Grade A-C) or equivalent.
  • I understand that I will be asked to provide original certificates to verify my qualifications at point of offer.
