Role summary
This is an exciting opportunity for a highly motivated Community Staff Nurse to join the Community Nursing Team in the North East locality of Shropshire. The role involves providing planned care to patients in their own homes or chosen place of residence, supporting timely discharge and offering an alternative to hospital admission. The team provides nursing care 7 days a week, from 8am to 6pm. Newly qualified nurses are encouraged to apply and will receive preceptorship and professional development support.
Essential criteria
- RGN
- Ability to demonstrate a level of English language and grammar commensurate with writing in service user case notes
- NMC Registered
- Previous community experience
- Post registration general experience
- Previous NHS experience
- Experience of working in a multi disciplinary / multi agency health/social care environment.
- Experience and knowledge of basic administration systems including Microsoft office
- Experience of successful partnership working arrangements with other services
- Knowledge and Skills
- I T skills or willingness to undertake Rio Training
- Up to date with Mandatory Training
- Uses evidence based practice
- Works within own defined competencies
- Awareness of changing healthcare factors influencing change
- Demonstrates tact and diplomacy
- Engaging and empowering approach that seeks to work with others and deliver outcomes in a successful and well informed manner

- Must be able to meet the travel requirements of the post
