Role summary
This role involves providing specialist nursing care to children and young people with complex and acute health needs across community, education, and clinic settings in Portsmouth and South-East Hampshire. You will manage a caseload, develop care plans, and deliver high-quality, family-centred care, working closely with families and multidisciplinary teams.
