Role summary

This role involves providing therapy-based support to individuals in community environments, helping them regain independence and achieve rehabilitation goals. As a Support Worker, you will play a vital role within a multidisciplinary team (MDT) dedicated to helping individuals regain independence and achieve their rehabilitation goals.

Your work will focus on providing therapy-based support in community environments, empowering clients to improve their functional abilities and quality of life. As well as working along side the community nursing team to provide treatment for patients within their own homes.

We are seeking candidates who are compassionate and patient with a strong desire to support others. Good communication and interpersonal skills are essential, with the ability to work independently (including lone working) and as part of the wider MDT team.
