Role summary
This role is for a Band 6 Community Mental Health Nurse within Herefordshire and Worcestershire Health and Care NHS Trust. The team primarily supports individuals over 65 with dementia or complex mental health issues across Worcestershire. Key responsibilities include undertaking urgent mental health assessments in the community, triaging referrals, and liaising with partner agencies and consultants. Essential criteria
- Documented evidence of continuing professional development
- Significant post registration experience including experience within mental health
- Minimum 1 year within / related specialty
- Extensive clinical experience including individual and group work
- RMN (Degree/Diploma and working towards degree)

- Specialist knowledge and application of interventions relevant to patient group

We have an opportunity for a full time band 6 mental health nurse. The Team works predominately with people aged over 65 who have dementia or complex mental health problems; the team covers all of Worcestershire.
The post involves working as a cohesive team to manage and triage the incoming referrals to the service, either signposting to other agencies or if appropriate assigning them to a community mental health team for assessment and treatment. The post also involves visiting patients that you have identified as requiring urgent input to complete an initial assessment and arrange appropriate follow up.
You will need to have experience with the patient group and community work, with a sound knowledge of risk assessment; you will be offered 1-1 supervision and ongoing support. You will enjoy working in a close-knit team and supportive team.
