Role summary
This is a Band 5 Community Learning Disability Nurse position within the Wrexham Learning Disability Community Health team. The post holder will work as part of the team, ensuring interfaces with local authorities and other services. They will deliver evidence-based, safe, and effective interventions and clinical care following the nursing process. The role involves delivering high-quality specialist services in line with agreed standards and communicating complex information.

- RNLD. Will consider student nurses due to qualify
- Experience in learning disability services
Desirable criteria
- Broad range of experience working in health & social care settings
- Demonstrate a good understanding of Learning Disability, including Mental Health, Forensics, Challenging Behaviour and PLD/Complex Health.-Awareness of current developments within a clinical setting
- Completed training in mentoring & Assessing student nurses
- Awareness of current developments within - Learning Disability services
- Understanding of clinical governance
- Values
- Evidence of effective team working
- Ability to work positively as part of a multi-disciplinary team
- Ability to work autonomously Display flexibility to adapt to organisational changes as well as changes within the service.
- Leadership and motivational skills
- Ability to prioritise and work proactively
- Ability to manage a designated caseload without direct supervision
- Ability to work as part of a team
- Flexible approach to the needs of the service

Job summary
This is an exciting opportunity to join the Wrexham Learning Disability Community Health team at Betsi Cadwaladr University Health Board.
This is a Band 5 Community Learning Disability Nurse position within Wrexham. The post holder will work as part of the Learning Disability team based at Wrexham ensuring interfaces between the Local Authority and neighbouring services within the Division including community mental health teams, in patient services, as well as other statutory agencies and the voluntary sector.
The post holder will ensure they deliver evidence based, safe effective interventions and clinical care, following the nursing process.
Main duties of the job
The post holder will ensure that they follow the service principles and strategy for people with a learning disability while delivering safe, effective and high-quality specialist service in line with agreed standards.
The post holder will have excellent communication skills and will be a visible and accessible member of the team, patients and their families can access for assistance, advice and support.
The post holder will be expected to communicate and receive highly complex and sensitive information and able to analyse and interpret information and act appropriately
