Role summary
This role is for an enthusiastic and experienced Band 6 Nurse to join a multidisciplinary team as a Frailty Practitioner within the Weymouth and Portland Integrated Community Service. The post holder will deliver high-quality, person-centred care for patients living with frailty, focusing on recognising and managing complex/deteriorating patients to prevent inappropriate hospital admissions. This involves working across community settings, providing specialist clinical input to optimise independence and improve patient outcomes. The practitioner will act autonomously while collaborating with the Integrated Neighboured Team and wider services to deliver coordinated care.

Desirable criteria
- Physical examination qualification
- Experience at band 6 level, or above
- Able to use own initiative and appropriate risk taking
- Experience and knowledge of working with frail patients
- Job specific experience
- Experience of managing change
- Demonstrable knowledge of assessment and therapeutic interventions in area of specialism
- Experience at band 6 or above
- Recent previous experience within a comparable community role
- Managerial/ Supervisory experience
- Experience of providing clinical supervision and mentoring to junior staff
- Experience of supporting and supervising learners
- Experience of devising and delivering training
- Information Technology / Resources
- Experience of using electronic patient / service user record systems
- Business Travel
- Subject to the provisions of the Equality Act, able to travel using own vehicle on Trust business.

Job summary
We are seeking an enthusiastic and experienced Band 6 Nurse to join our multidisciplinary team as a Frailty Practitioner. This is an exciting opportunity to work within the Weymouth and Portland Integrated Community Service to support Frailty Hospital At Home and the Complex Frailty MDT function.
The post holder will play a key role in delivering high-quality, person-centred care for patients living with frailty, recognising and managing complex/deteriorating patients and to prevent inappropriate admission to acute trusts.
This will involve working across community settings, providing specialist clinical input to optimise independence, reduce hospital admissions, and improve patient outcomes.
