Role summary
This is an exciting opportunity for a Community Staff Nurse to join the South Shore Nursing Team. The role involves providing high-quality, patient-centred nursing care in patients' homes or clinic settings across the Fylde coast. You will assess, plan, implement, and evaluate individual care requirements, working collaboratively with patients, families, and the wider team. A full UK driving licence and access to a car are essential for this role.
Essential criteria
- Registered Nurse level 1
- Evidence of ongoing professional development
- Current NMC registration
- Supervisor/Assessor competence or willingness to undertake
Desirable criteria
- V150/V100/V300 Prescriber
- Evidence of CPD - Management of Leg Ulcers CPD Module
- Evidence of CPD - Tissue Viability CPD module
- Experience and Knowledge
- Post registration experience
- Able to deal with difficult situations
- Ability to Identify and assess patients' needs
- Ability to use resources appropriately
- Multidisciplinary working
- Willingness to undertake further training specific to role
- Previous community nursing experience
- Skills and Ability
- Good team worker.
- Ability to organise self and others by prioritising and delegating as needed
- Good time management skills.
- Ability to educate others
- IT literate
- Clinical audit knowledge or ability to participate.
- Willing to develop professionally.
- Up to date knowledge of current NHS issues and aware of current trends in the NHS

Main duties of the job
The postholder will work as part of a community nursing service working across the Fylde coast, undertaking nursing duties in patients' homes or clinic settings.
To provide safe, effective, individualised care to patients and to give support to families and service users requiring scheduled and unscheduled care interventions.
The role includes planning, implementing and evaluating programmes of care, initiating changes to care and escalation, if required, to the senior community nurse and the wider team.
To monitor the condition and health needs of patients on a continual basis in partnership
