Role summary
This role involves assessing, planning, coordinating, and reviewing care for adults with learning disabilities who may have complex physical and mental health needs. The Community Nurse will work within a multidisciplinary team across Plymouth City, liaising with various healthcare and social care agencies. Key responsibilities include providing clinical support, promoting high-quality person-centred care, and participating in Continuing Healthcare funding reviews. The position requires working across a 7-day service.
Key responsibilities
- Assess, plan, coordinate, and review care for adults with learning disabilities.
- Provide clinical support and promote high-quality, outcome-focused service.
- Participate in reviewing eligibility for Continuing Healthcare funding.
- Identify, plan, and manage risks related to individuals' care needs.
- Maintain effective communication networks with multidisciplinary teams and stakeholders.
- Act as a supervisor and/or assessor for students.
Essential criteria
- Working in multidisciplinary teams and across health and social care.
- Managing a complex caseload of individuals who pose a risk to themselves and society.
- Experience of working across organisational and inter-agency boundaries.
- Experience of working with people with learning disabilities / mental health issues whose behaviour poses a risk.
- Able to demonstrate understanding and competency in assessment, care planning and review.
- Good knowledge and understanding of the, Mental Capacity Act, Deprivation of Liberty and reasonable adjustments.
- A good understanding of the safeguarding adult and childrens processes and relevant issues.
- A knowledge of the Mental Health Act.
- Able to problem solve and be creative in care planning
- Knowledge of Continuing Healthcare Processs
- Able to engage effectively in dialogue with individuals at all abilities effectively managing barriers to communication
- Ability to take positive risk and work proactively with clients who display behaviours of concern
- Excellent interpersonal skills
- Ability to prioritise against tight deadlines.
- Good negotiation skills
- Excellent organisational skills and team working.
- Able to present relevant information in a concise format.
- Able to write in plain, easy and understandable style
- Capacity to work with clinicians and staff at all levels to develop a shared vision of outcomes for individuals.
- Registered Nurse RNLD/RMN
- Relevant post registration experience.
- Evidence of continuing professional development.
- Evidence of managing a caseload of complex individuals.
- Ability to frequently travel independently and in a timely manner between Livewell Southwest sites and community locations including areas not served by public transport.
- The ability to be away from home overnight is an integral part of the role.
- Able to problem solve and be creative in care planning.
Desirable criteria
- Experience in mentoring students.
- Working in close contact with ICB and local commissioners.
- Knowledge and understanding of Ministry of Justice restrictions/Forensic Mental Health.
- Mentorship Certificate.
- Leadership qualification/knowledge.

Job summary
22.5 hours per week.
The Community Nurse is responsible for assessing, planning, coordinating, and reviewing a caseload of adults with learning disabilities. They may have either or both physical and mental health complex care needs. This will include relationships with Primary and Secondary health care, as well as private and voluntary care agencies.
The Community Learning Disability Service covers Plymouth City offering a specialist service to Adults with a Learning Disability registered to a Plymouth GP surgery and comes under the City-Wide Locality Services.
The service is led by the Service Manager and 2 Group Managers and professional leads. We focus on Multi-Disciplinary/Professional led care. The team provides Consultant Psychiatry care alongside Psychology, Nursing (learning Disability and Mental Health), Speech and Language, Occupational Therapy, Dietician, Positive Behavioural Support, Health Education and Promotion, Primary Care Liaison Nurses and Community Support Workers.
Main duties of the job
To be responsible for providing clinical support whilst promoting the effective and efficient delivery of high-quality outcome focused service in a person-centred approach.
This is a generic community care role providing support and cover across Plymouth city localities.
