Role summary
This role is for a Band 6 Community Mental Health Practitioner (Nurse, Social Worker, or Occupational Therapist) within the Chichester Neighbourhood Mental Health Team. The practitioner will manage a caseload of service users with complex mental health needs, providing high-quality, evidence-based assessment, treatment, and care. Responsibilities include collaborative care planning, therapeutic interventions, and risk management, working closely with service users, carers, and other agencies to promote recovery and independence. The position requires a compassionate, adaptable practitioner with excellent communication and clinical decision-making skills.
Key responsibilities
- Undertaking comprehensive assessments of service users, including those with complex mental health needs and increased risk.
- Developing, implementing, and reviewing collaborative, strengths-based care and recovery plans with service users.
- Providing specialist nursing assessments, interventions, and clinical advice to service users, carers, and multidisciplinary colleagues.
- Assessing, managing, and regularly reviewing identified risks, developing effective risk management plans.
- Delivering evidence-based clinical interventions consistent with best practice, NICE guidance, and Trust policies.
- Providing clinical supervision, mentorship, and support to junior staff, students, and trainees.
Essential criteria
- Registered Mental Health qualification or equivalent
- Up to date Registration
- Post Basic Courses / relevant Study Days
- Evidence of CPD
- post-qualifying experience of clinical work with people with a range of mental health problems
- Experience of partnership working within and across various different statutory and non- statutory teams and agencies
- Experience of successfully managing a caseload and prioritising clinical work
- Experience of providing a range of clinical interventions to people with a variety of mental health problems.
- Experience of working consultatively with professionals
- Experienced in conducting clinical assessments, including risk assessments.
