Role summary
This is an exciting opportunity for a Band 6 Clinical Practitioner to join the Urgent Community Response Team in West Essex. The role involves assessing and managing acutely unwell patients at home to prevent hospital admissions and facilitate early discharge. You will deliver high-quality care, conduct holistic assessments, and implement care plans within the Hospital at Home and UCRT pathways. The position requires an enthusiastic and motivated individual with strong communication skills and a commitment to professional development.
Key responsibilities
- Prioritise and plan own workload with some direction.
- Manage patient care autonomously using advanced knowledge and skills.
- Ensure assessments and care plans comply with Trust clinical standards.
- Provide home visits and deliver care according to clinical need.
- Facilitate safe and timely discharge from the service, including generating discharge summaries to GPs.
- Recognise deteriorating patients and use appropriate escalation pathways.
Essential criteria
- NMC registration or AHP registration
- Car Driver
- Demonstrates evidence based clinical knowledge
- Has substantial post registration experience including work within chosen speciality
- Have knowledge of current professional matters in relation to the NHS.
- Ability to communicate effectively with patients from varied social and cultural backgrounds.
- Communicates succinctly, clearly and accurately both verbally and in writing
- High standards of written/computer skills.
- Willingness to uphold the Trust's and provider's values
- Eligible to live and work in the UK.
- Be an effective team member who is able to relate to colleagues at all levels of the Trust with tact and diplomacy
- Possesses a valid UK driving license and access to a vehicle for work purposes
Desirable criteria
- Clinical experience in Community Care
- SystmOne experience or similar electronic patient record systems

Job summary
An exciting opportunity has arisen for Band 6 practitioners to work within our established Urgent Community Response Team in West Essex.
The teams assess and manage acutely unwell patients at home using a variety of pathways including respiratory, heart failure and frailty, to prevent unnecessary admissions to hospital.
This includes the utilisation of remote technology and new ways of working to provide a safe alternative to hospital admission and support people to be discharged from hospital earlier than may have been possible previously. The focus of this role is delivering high quality care in the community, holistic assessments, planning, implementing, and reviewing care provision across the Hospital at Home and UCRT pathways of care. This is an exciting opportunity to shape services that will improve the experience of healthcare for the population of West Essex and deliver an NHS national priority.
