Role summary
The Triage Nurse provides clinical triage assessment and support for healthcare professional referrers, assisting with admission avoidance and hospital discharges. This role acts as the access point for all community nursing referrals, offering immediate clinical advice or signposting to appropriate services. The nurse will work independently, exercising assessment and evaluation skills to take comprehensive referrals for community nursing teams. Excellent communication and professional judgment are essential to maintain links with health and social care professionals.
Key responsibilities
- Provide triage assessment, clinical support, and professional expertise to healthcare professional referrers.
- Act as the access point for all community nursing referrals for the Locality referrals professionals.
- Provide high quality, clinical triage using knowledge, skills and critical thinking and professional judgement.
- Control and manage calls effectively by using skilled questioning, active listening and excellent verbal communication skills.
- Work as a member of the nursing team in conjunction with other professionals to promote seamless patient care.
- Be personally accountable for professional and ethical actions and omissions in compliance with the NMC Code of Conduct.
Essential criteria
- Current registration with NMC
- Registered Adult Nurse
- Evidence of continued professional development relevant to role
- Hold the ability to demonstrate sound knowledge and application of up to date evidence based clinical practice
- Excellent communication skills
- Effective time management with the ability to meet competing deadlines
- Competent IT skills working across a variety of systems
- Flexibility with working hours to enable 7 day working.

The Triage Nurse will provide triage assessment, clinical support, assistance and professional expertise to all health care professional referrers requiring assistance and support with Admission Avoidance and hospital discharges.
