Role summary
This role is for a newly qualified or experienced Occupational Therapist to join a new Intensive Case Management (ICM) and Rehabilitation Pathway in Buckinghamshire. You will work with adults and older adults with complex mental health needs, supporting their independence and recovery within their communities. The role involves delivering person-centred, recovery-focused OT interventions, primarily in people's homes and local communities, as part of a multidisciplinary team. You will manage your own caseload, receive supervision, and contribute to service development.
Key responsibilities
- Delivering high-quality Occupational Therapy assessments and interventions for adults with complex mental health needs.
- Supporting people to develop the skills, confidence and independence needed to live fulfilling lives within their communities.
- Providing assertive outreach by meeting people where they feel most comfortable, including their homes and community settings.
- Building therapeutic relationships with individuals who may find traditional services difficult to engage with.
- Working collaboratively with colleagues across health, social care and voluntary services to provide joined-up care.
- Contributing to recovery planning, risk management and discharge planning.
- Maintaining accurate clinical records and participating in service development, quality improvement and audit.
Essential criteria
