Role summary
This is an opportunity for an enthusiastic and highly motivated Diabetes Specialist Nurse to join the innovative Barts Health NHS Trust Young Adults Diabetes Service. The post holder will support young adults (age 16-25) with diabetes, including those struggling with difficulties related to living with diabetes, as they transition to adult services. The role involves working with both Type 1 and Type 2 diabetes patients across all Barts Health sites, providing specialist care and education. The successful candidate will develop management plans and treatment advice, working as part of a multidisciplinary team.
Key responsibilities
- Support high risk young adults and work with the team's social worker, youth worker and psychologist to safeguard and engage these patients.
- Develop specialist skills to provide management plans, treatment advice, and support for young adults (age 16-25) with diabetes and their carers.
- Work across all sites providing specialist care and support for young people, their families and site-based clinical diabetes teams.
- Act as a clinical and educational resource for patients and health professionals.
- Be a good team player, always striving to improve the care of people with diabetes.
Essential criteria
- First level NMC registration
- Mentorship qualification
- University diabetes course or equivalent level of knowledge or experience
- Post-registration experience
- Experience of working with young people with diabetes
- Experience of supervision of junior staff / nursing students
- Experience of patient education to support self-management
- Experience working with diabetes technology
- Awareness of the structure and organisation of the wider NHS
Desirable criteria
- Teaching and assessing (ENB998) or equivalent level of knowledge or experience
- Graduate in nursing or health related studies
- Recognised certificate in counselling
- Participation in Nursing Research and Audit
- Experience in the management of diabetes in the community or primary care or of working on a diabetes ward

Job summary
An opportunity has arisen for an enthusiastic and highly motivated DSN to join the innovative Barts Health NHS Trust Young Adults Diabetes Service. This mobile service will also comprise a Dietician, Clinical psychologist, Youth Worker, Social Prescriber, and administrator.
This exciting new post has been developed to improve diabetes care and outcomes for young people (age 16-25) with diabetes across North East London. The post holder will support young adults who may be struggling with a range of difficulties related to living with diabetes, as they make the transition to adult services. The work will be with young adults living with both Type 1 and Type 2 diabetes.
The post holder will have scope to develop the service, with the support of more senior colleagues and in collaboration with service users.The post will have a base at Mile End Hospital, but the job will involve working across all of the Barts Health sites (Whipps Cross Hospital, Newham University Hospital, the Royal London Hospital, and St Barts Hospital), and ensuring that standardised, high-quality care is delivered across the Trust.
