Role summary
This patient-facing role involves joining the Emergency and Acute Care (EAC) directorate's pharmacy team, specifically supporting the Emergency Department and Same Day Emergency Care (SDEC). You will work alongside the clinical pharmacy team, focusing on medicines reconciliation, patient counselling, and managing ward automation systems. The position includes leadership and clinical responsibilities, assisting in service improvements and ensuring a safe, efficient, high-quality, and cost-effective technical service. You will be supported by experienced pharmacists and technicians.
Key responsibilities
- Compile patient medication histories by checking various sources like patient records, smart cards, and GP surgeries.
- Communicate complex or sensitive information effectively to staff, healthcare professionals, and patients.
- Check patients' own medicines for suitability on admission and discharge, and complete final accuracy checks of discharge prescriptions.
- Order further medication supplies as per departmental procedures and maintain up-to-date handovers on NerveCentre.
- Liaise with pharmacy stores to manage drug supply issues and participate in controlled drug retrieval, destruction, and auditing.
- Implement and maintain standards for medicines storage and security, including temperature monitoring and security of various drug storage locations.
Essential criteria
- Experience in the admission/discharge processes within hospital pharmacy
- Proven knowledge and experience in using computer labelling and stock control systems within hospital pharmacy
- Can demonstrate that they can work on their own initiative but know limits of their authority and knowledge
- Ability to perform and work under pressure to deliver objectives and plans
- To communicate effectively, clearly and concisely complex or sensitive information relative to medicine management back to patients, staff and colleagues
- Good interpersonal skills with the ability to liaise with a wide range of staff and patients
- Proven evidence of judgement and prioritisation skills with regards to targets, deadlines and stock management.
- Accurate with good attention to detail
- Experience in organising workload to meet pharmacy services and patients' needs
- Self-motivated and able to motivate others
- Approachable
- Competent ICT skills
- Sound Word, Excel skills and be able to retrieve information from the internet
- Proven evidence of judgement and prioritisation skills with regards to targets, deadlines and stock management
- Proven experience in problem solving
- Accurate with good attention to detail
- Qualified Pharmacy Technician registered (or has ability to register) with the General Pharmaceutical Council
- Accuracy Checking Pharmacy Technician (Accredited scheme or evidence of in-house validations).
- Knowledge of medicines management, including relevant legislation and policies plus specialist training and experience

Job summary
We are delighted to invite applicants to join our Emergency and Acute Care (EAC) directorate and become part of the Emergency Department and Same Day Emergency Care (SDEC) pharmacy team.
This post is patient-facing and involves working alongside our established clinical pharmacy team to deliver a specialised service to our ED and SDEC. You will have a key role in supporting with medicines reconciliation, counselling patients and working with the team to manage the ward automations systems, helping to ensure the effective running of the Pharmacy support with the Emergency Department and SDEC.
