Role summary
Join Frimley Health NHS Foundation Trust as a Band 3 Support Worker in the Community Bridging Service. This role involves providing high-quality support to clients in their own homes, assisting with daily living activities, and promoting independence. You will work shifts between 8am and 9pm, including weekends, and play a vital role in early identification of deteriorating patients. A full UK driving licence and business insurance are essential for this position.
Key responsibilities
- Provide high quality care to clients under the indirect supervision of Team manager within the community bridging service.
- Support clients with activities of daily living in their home.
- Ensure high standards are maintained in a safe environment, which promotes equality and sensitivity for all individuals.
- Participate in continuous professional development and other team activities.
- Carry out a range of duties within agreed competency skill set e.g. meal prep.
- Ensure that all patients on the caseload are safe.
Essential criteria
- Level 2 Apprenticeship in Care (or relevant subject) or a minimum of 1yr experience in health and social care setting
- GCSE maths and English (or equivalent)
- Previous experience in care eg as HCA
- Safe moving and handling experience
- Moving and handling of clients
- Personal care to clients in a person-centred manner
- Follow clear instructions and feedback to senior staff
- Work safely with minimal supervision
- Hold a full UK Driving Licence
Desirable criteria
- Level 3 Apprenticeship in Care (or relevant subject)
- Care certificate
- Previous community experience
- Complex moving and handling experience
- Knowledge of some medical conditions / exercise therapy
- Good interpersonal and communication skills both verbal and written

This vacancy is for 2 Part Time Positions.
Post 1 - 28 hours per week (3x 6hr Shifts and 2x 5 hour shifts)
Post 2 - 24 hours per week (2x 6 hour shits and 3x 4 hour shifts)
Early shifts start at 8am so you may be working 8-12, 8-1 or 8-2 depending on your shift pattern. Late shifts finish at 9pm so you may be working 4-9 or 5-9 depending on your shift pattern.
