Role summary
This role involves being the first point of contact for individuals seeking mental health support within the Portsmouth Mental Health Hub. You will conduct person-centred telephone assessments, identify risks, and develop collaborative care plans. The facilitator will connect people with appropriate mental health, housing, financial, community, and wellbeing services. It requires working autonomously and closely with various healthcare and voluntary sector colleagues to support recovery and prevent mental health crises.
Essential criteria
- NVQ 3 or Health Related Qualification or equivalent experience
Desirable criteria
- Experience with working with people and patients, carers.
- Experience of working within a multidisciplinary team.
- To have a working knowledge of Risk Assessment and management tools currently in used within acute mental health settings.
- Experience in an acute / crisis mental health setting

Job summary
Are you passionate about supporting people with their mental health and helping them access the right support at the right time? We are looking for a compassionate and motivated Mental Health Pathway Facilitator to join Portsmouth Mental Health Hub.
In this rewarding role, you will be the first point of contact for individuals seeking mental health support. Working autonomously, you will undertake person-centred telephone assessments, identify risks, develop collaborative care plans, and connect people with appropriate mental health, housing, financial, community and wellbeing services. You will work closely with colleagues across primary care, mental health services and voluntary sector organisations to support recovery and prevent mental health crises.
We are seeking someone with excellent communication and listening skills, experience working with service users, carers or patients, and a genuine commitment to person-centred care. Knowledge of mental health and risk management is essential, as is the ability to work flexibly in a fast-paced environment.
