Role summary
This role is for a dynamic and enthusiastic Rehabilitation Assistant within the Early Intervention Community Team at Moseley Hall Hospital. The team focuses on preventing hospital admissions and facilitating discharges. You will work alongside nurses and therapists to provide innovative care to adults in the community, contributing to comprehensive care plans. The position requires flexibility, including working shifts to provide a seven-day service between 8 am and 10 pm.
Key responsibilities
- Perform a pivotal role within the Early Intervention Community Team.
- Provide innovative service to adults within the community.
- Contribute to comprehensive care plans for individual service users.
- Work alongside Nurses and Therapists to prevent avoidable hospital admissions and facilitate discharge.
- Work flexible shifts to provide a seven-day service between 8 am and 10 pm.
Essential criteria
- English and Maths GCSE/Good standard of education
- NVQ level 3 in Health and Social Care or equivalent experience or evidence of completion of in house further opportunities programme
- Relevant experience in delivering direct patient care in a health or social care setting.
- Effective oral and written communication skills
- Basic IT Skills
- Able to work in a team
- Able to work independently
- Awareness of the needs of a multicultural, socially diverse population.
- Awareness of responsibilities related to Safeguarding Vulnerable adults and children
- Good interpersonal skills
- Demonstrates awareness of the needs of individuals' minority groups
- Ability to work Flexible hours eg Weekends / Bank Holidays
Desirable criteria
- Experience of working in a rehabilitation setting.
