Role summary
This is an exciting opportunity for an experienced registered mental health nurse to work in Primary Care in the Warrington area, covering several GP surgeries. The role involves providing specialist mental health assessments for patients with moderate to severe conditions, bridging the gap between Primary and Secondary Care Mental Health Services. Key responsibilities
- Provide safe and effective day-to-day operation of the service, including handling clinical enquiries and undertaking telephone triage screening assessments.
- Undertake assessments of individual patients' difficulties, establishing risk and determining appropriate courses of action, engaging service users in their health and wellbeing.
- Work with patients to support adherence to prescribed treatments, monitoring mental health medication, and considering physical health interactions.
- Provide short-term interventions and support under GP guidance, ensuring monitoring and treatment align with practice standards.
- Collate and organise information about local opportunities and resources, facilitating signposting to other appropriate services and undertaking short-term interventions for patients with moderate to severe mental health difficulties.
- Evaluate outcomes, maintain accurate clinical data, and ensure practice is within clinical competencies and knowledge base, seeking support as required.
